Transition Services
Transitioning from children’s
services into adult services for a youth with
special needs and their family often presents
significant challenges and barriers. This journey
can be very exciting, yet can also feel overwhelming
at times. To meet the transition challenge,
the Children’s Link Society has developed
Transitional Support Services to empower and assist
families who are beginning or are in the process of
transitioning into adult services.
Transitional Support Services provide information
and support to individuals with disabilities and
their families who may be uncertain of the
transition process or want to know more about the
adult services which may be accessed by their youth
in the future. At the Children’s Link, a
Transitional Support Work is available to answer
your questions and provide family centred support
during the transition into adult services.

Assistance in the
coordination of services

Assistance in applying for
AISH (Assured Income for the Severely
Handicapped) & PDD (Persons with Developmental
Disabilities)

Supporting individuals in
filling out service application forms

Providing unbiased and
current information about adult Service
Providers

Other supports and
information which may be required to
successfully transition from Children’s services
to adult services
